Age Limit:

The Candidate should not be less than 18 years of age and not more than 38 years of age as on 01-01-2021. The upper age limit is relaxable:-

a. By 5 years for SC/ST candidates, i.e up to 43 years.
b. Bv 3 years for OBC/MOBC candidates i.e up to 41 years.

Resident-ship: The candidate must be a citizen of India.

Physical Fitness:

(a) The candidate must be of sound health, both mentally and physically.
(b) Required to undergo medical examination before appointment to the service.

> The eligible candidates can submit/ address their application to the Office of the Addl. Chief Medical & Health Officer (FW), Rong Khela, Diphu in the DropBox from 12th to 29th November 2021 during office hours.

> The application should clearly write “APPLICATION FOR THE POST OF” on the front cover of the envelope.

> The eligible candidates shall apply in a standard form of application along with a KAAC money receipt of Rs. 10/- (Ten) as application fee. The eligible candidate must submit 3 (Three) Nos. of Passport size photograph affixing one, self-attested copy on the application form.

> Self-attested Xerox copy of all relevant certificate/ Mark sheets/ caste certificates etc. have to be submitted along with the application.

> The candidates willing to apply for more than one post shall have to submit a separate application form for each post.
